Lead client implementations of Carefeed’s Referral Management module for skilled nursing and post-acute care organizations. Assess referral, authorization, intake, documentation, and concurrent-review workflows; configure solutions; train cross-functional client teams; manage milestones through go-live; develop implementation resources; and partner with Product, Client Experience, and Support. The role requires extensive skilled nursing operations expertise, payer coordination knowledge, and the ability to translate clinical and operational processes into effective technology workflows.
