Provides and coordinates skilled nursing care to patients in their homes. Responsibilities include comprehensive assessments, OASIS completion, medication reconciliation, care-plan development, supervision of aides and LPNs, documentation, physician communication, staff precepting, quality improvement, patient safety, and on-call or weekend participation.

Oversee day-to-day operations of a home health agency, including patient eligibility, care planning, regulatory compliance, profitability, staffing, retention, and service quality. Coordinate strategic projects, maintain administrative policies, supervise home healthcare delivery, and collaborate with company management and departments. The role requires unrestricted RN licensure, supervisory or administrative experience, reliable transportation, and strong organizational and communication skills.

Provides skilled nursing care to patients in their homes according to state scope of practice and individualized care plans. Coordinates with the healthcare team, educates family members, monitors patient responses and condition changes, and reports findings to supervising nurses or physicians. Requires unrestricted LPN/LVN licensure, reliable transportation, and a driver’s license. CPR certification, home care experience, flexibility, independence, and strong communication skills are preferred.
