Day in the Life Value Based Programs Specialist

This role will assist in driving comprehensive care for Crossroads patients enrolled in value-based arrangements. This role will conduct end-to-end analyses and evaluations of programs and processes, bringing together facts, analysis, and judgement to solve complex problems to ensure targets for value-based quality measures are achieved.

  • Support the development of compelling, logically structured communications, presentations and deliverables that translate data into clear, actionable insights
  • Collaborate with cross-functional teams to design, implement, and monitor value-based care initiatives.
  • Conduct in-depth analyses to evaluate the effectiveness and efficiency of value-based programs, identifying areas for improvement and recommending actionable solutions.
  • Utilize data-driven insights to inform strategic decision-making and enhance program performance.
  • Identify and escalate risks to the Sr. Manager of Value Based Programs while supporting the development of mitigation strategies.
  • Support the development of systems and procedures within Crossroads operational and medical operational teams to take action, when appropriate, within populations of specific patients
  • Synthesize data to evaluate patterns and opportunities based on locations, demographics, and other categories. Recommends actions for continuous improvement.
  • Provide expert guidance and support to healthcare teams, facilitating the adoption of value-based care practices.
  • Assist with building and maintaining data systems to track specific actions and hold teams accountable for results and metrics linked to measures defined by individual contracts and plans.
  • Stay informed about industry trends, regulatory changes, and best practices in value-based care to continuously refine program strategies.

Location

  • Remote with travel to Corporate (Greenville, SC) monthly (one week a month Monday through Thursday) to meet with team.

Education and Requirements

  • Bachelor’s degree in Healthcare Administration, Data Analytics, Business Administration, or related field; or a combination of education and equivalent work experience required.
  • 1-3 years of Health Administration, Data Analytics, management/strategy Consulting, or other related field experience preferred.
