LOCATION: Remote – must live in North Carolina or within 40 miles of the North Carolina border. Travel is expected and required for this role.
GENERAL STATEMENT OF JOB
The Rural Health Transformation Program (RHTP) is statewide program administered by the NC Department of Health and Human Services (DHHS). The purpose of the RHTP is to create sustainable improved access and outcomes to NC citizens living in rural communities. This program is funded by the US Centers for Medicare & Medicaid Services (CMS).
The RHTP ROOTS Hub Director reports to the Executive Vice President and Chief Operating Officer. The ROOTS Hub Director will provide strategic direction and ensure the effectiveness of Hub Lead operations including fiduciary oversight, grantmaking and reimbursement processing, compliance and monitoring, Hub-wide reporting, and implementing the Hub Action Plan in NC DHHS’ defined region 3 comprised of Alexander, Anson, Cabarrus, Catawba, Cleveland, Gaston, Iredell, Lincoln, Mecklenburg, Rowan, Stanly and Union counties. ROOTS Hub activities will focus on rural counties within region 3, while this does not preclude cooperation or expansion of efforts to urban counties where priorities align, funding will be prioritized for rural counties. The position is responsible for the daily operations of the Hub, ensuring the effectiveness of the program and provides direct supervision to ROOTS Hub Connectors, Lead Implementation team, Performance Reporting Analyst, Financial Analysts, and Quality Improvement Specialist. The ROOTS Hub Director will work directly with NCDHHS, Hub Governing Body, Regional Decision Councils, and Technical Advisory Panel.
The ROOTS Hub Director is responsible for close collaboration with region 3 lead anchors (App State/AppWell/Office of Rural Promise, Partners Behavioral Health Management, Cabarrus Health Alliance and Catawba County Public Health), tribal representatives where applicable and tailoring NCRHTP initiatives and activities to meet region 3’s rural health needs and the NCRHTP performance objectives:
- Build rural community hubs
- Create models and capacity for expanded primary care and prevention and chronic disease management
- Build a robust workforce and care team models
- Ensure fiscal sustainability of rural health providers through innovative fiscal models
The ROOTS Hub Director will play a critical role in ensuring the effectiveness of the Rural Health Transformation Program. This position is responsible for oversight and ensuring the effective implementation, monitoring, reporting, and administrative work related to the launching and daily operations of the NC RHTP. This position requires the following skills: high degree of emotional intelligence, discretion, independent judgment, strong written and verbal communication skills tailored to a wide range of stakeholders and community partners, strong project management and performance improvement skills, and experience in managing large scale multi-pronged implementation plans and budgets.
ESSENTIAL JOB FUNCTIONS
Requirements from NC DHHS:
- Serve as Vaya’s RHTP liaison with the Department and other awarded ROOTS Hub Lead entities on the RHTP program-related roles and responsibilities
- Oversee and ensure the successful implementation and compliance with NC DHHS scope of work and Vaya’s RHTP plan for Initiatives 1,2, 4 and 5 in NC DHHS defined region 3. Initiatives at a high level include the following:
- Initiative 1 – Establish ROOTS Hub that connects medical, behavioral health, and social services to improve availability and access to healthcare in rural communities
- Initiative 2 – Improve prevention/screening, chronic disease management, maternal and nutritional health by scaling up effective programs
- Initiative 4 – Modernize and sustain rural workforce through investment in fellowships and certification programs to recruit, train and retain clinicians, allied health and community health workers
- Initiative 5 – Advance value-based payment capabilities for rural primary care and rural health systems
- Coordinate within the organization and with lead anchor partners to ensure all RHTP responsibilities and requirements are met\Track Vaya’s RHTP plan’s compliance and performance against program-related deadlines and milestones
- Submit program-related reports to the Department by required deadlines
- Support subrecipients, implementation partners, and community-based organizations in understanding contracting, invoicing, documentation, reimbursement, reporting, and compliance expectations.
Hub Operations, Partner Coordination, and Implementation:
- Provider leadership and coordination of internal RHTP team
- Partner closely with project team, finance department, lead anchors and community stakeholders to ensure the milestones and deliverables for RHTP
- Facilitate Hub governance operations, including preparation of agendas, meeting materials, decision logs, feedback-to-decision documentation, escalation items, and follow-up actions for the Hub Governing Body, Regional Decision Council, and related advisory or review functions.
- Oversee the operational administration of the Hub’s intake, screening, merit review, technical advisory review, regional recommendation, and final funding approval processes, including coordination of reviewer rosters, scoring tools, conflict-of-interest documentation, recommendation packets, and procedural integrity.
- Maintain the Region 3 ROOTS risk register and oversee risk identification, mitigation, corrective action tracking, and escalation of fiscal, operational, compliance, partner performance, or implementation risks.
- Coordinate with Finance, Legal, Contracts, Compliance, and program staff to support subaward execution, invoice review, budget-to-actual monitoring, reimbursement documentation, and partner fiscal accountability.
- Ensure Hub implementation reflects community voice, rural priorities, lived experience, disproportionately burdened communities, and regional needs assessment findings, including documentation of how community feedback informs Hub priorities and implementation decisions.
- Maintain project plan and provide regular updates to supervisor and Vaya leadership regarding RHTP successes and challenges
- Provide and coordinate RHTG trainings as needed
- Attend and/or lead RHTP related meetings
- Maintain all plans associated RHTG initiatives 1,2,4 and 5.
- Serve as RHTP Subject Matter Expert (SME) as requested
- Participate in community health assessments to support the development of needed services
KNOWLEDGE, SKILLS, & ABILITIES
- Ability to lead, coach and support direct reports and lead by influence for indirect reports and partner agencies
- Knowledge of daily operations of the RHTP
- Knowledge of the RHTP initiatives, NC DHHS contract requirements and Hub Lead Plan
- Knowledge of healthcare systems, payers, partners and components needed to develop and implement a cohesive RHTG strategy
- Proficiency in evaluating and communicating successes and barriers with the RHTP initiatives
- Ability to pay close attention to detail and translate that into meaningful follow up/next steps
- Initiative-taking, collaborator, self-starter and able to work independently with little or no direction
- Elevated level of knowledge of Microsoft Office applications and adept at learning software applications
- Ability to exercise discretion and make independent decisions without supervisory input on matters of significance
- Knowledge of the interaction between mental health, physical health, and social determinants of health and quality health outcomes/well being
- Strong customer service skills paired with a positive demeanor – a “can do” attitude
- Prior demonstrated success in stakeholder relations

EDUCATION & EXPERIENCE REQUIREMENTS
Bachelor’s degree required. Master’s degree preferred.
- Must reside in North Carolina and be hired no later than June 1, 2026 [proposed date]
- Minimum of two (2) years of experience project managing large and complex engagements
- Minimum of two (2) years of experience coordinating across different types of stakeholders
- Minimum of two (2) years of experience in an organization or field that demonstrates an understanding of the healthcare systems, Medicaid and/or Medicare payer systems, clinical and social needs on communities’ and/or populations’ access to health services.
Preferred work experience:
Minimum of two (2) years of experience serving or working on behalf of Medicaid and/or Medicare beneficiaries. Population health experience and experience with physical health, behavioral health, I/DD and TBI populations preferred.
Licensure/Certification Preferred:
- Certifications in use of project management methodologies preferred
- Certification in quality and process improvement preferred
PHYSICAL REQUIREMENTS
- Close visual acuity to perform activities such as preparation and analysis of documents; viewing a computer terminal; and extensive reading.
- Physical activity in this position includes crouching, reaching, walking, talking, hearing and repetitive motion of hands, wrists, and fingers.

RESIDENCY REQUIREMENT: The person in this position is required to reside in North Carolina or within 40 miles of the North Carolina border.
SALARY: Depending on qualifications & experience of candidate. This position is exempt and is not eligible for overtime compensation.
