Habitat Health empowers older adults to experience more good days in their homes and communities. Through the Program of All-Inclusive Care for the Elderly (PACE), we provide comprehensive medical care along with support for daily needs such as meals, transportation, and inhome assistance. We deliver coordinated clinical and social care in our centers and directly in participants’ homes, creating a fully integrated experience that brings peace of mind and a true sense of belonging. As we expand our scalable, affordable PACE model to meet the growing and complex needs of aging populations, our missiondriven care teams continue to help participants live well on their own terms.

Habitat Health is supported by leading healthcare organizations and investors including New Enterprise Associates, Kaiser Permanente, and Town Hall Ventures. We are entering a period of significant growth and are looking for exceptional teammates to help us scale a better model of care for older adults. To learn more, visit www.habitathealth.com.

The Director of Strategic Operations is a high-impact, high-visibility role at the core of how Habitat operates. This role is responsible for driving Habitat’s operating model and bringing structured, strategic thinking to our highest priority, cross-functional initiatives. You will own the end-to-end launch process for new PACE centers, facilitate effective change management across the enterprise, and build the execution infrastructure that will enable Habitat to grow at scale.

This is a role for someone who thrives in ambiguity, operates with the rigor of a consultant and the urgency of an operator, and can move seamlessly between strategic design and hands-on delivery. This individual and their team will drive effective and efficient maturation of Habitat’s operating model.

Location: San Francisco, CA preferred, remote with 25-30% travel

Core Responsibilities & Expectations for the Role:

Center Launch Readiness

  • Lead Go-Live Readiness (GLR) for our new PACE centers — owning the strategy, operating model, milestones, governance, and cross-functional execution across clinical, operational, regulatory, staffing, tech, and other workstreams
  • Run executive readiness reviews and cross-functional forums to drive alignment and decision-making
  • Proactively surface and resolve risks, dependencies, and execution gaps across all launch workstreams
  • Deliver executive updates about launches to key Habitat and partner stakeholders
  • Manage the structured handoff to local operations teams once a center is live
  • Refine Habitat’s launch framework with each iteration

Change Management and Execution Infrastructure

  • Define how Habitat manages large-scale change — designing rollout strategies, governance structures, and operating rhythms that drive adoption and sustain results
  • Proactively identify and resolve friction points across teams, workflows, and priorities
  • Build repeatable systems and ways of working that strengthen Habitat's execution capability as we scale

Business Operations

  • Define and manage the operational metrics that matter to provide the right visibility into Habitat’s performance
  • Establish clear prioritization escalation pathways so that risks, blockers, and decision points surface to the right people at the right time
  • Translate data and status into effective executive-level reporting that enables timely, informed decision-making

High-Priority Cross-Functional Initiatives

  • Own a portfolio of strategic initiatives — spanning AI enablement, program design, and vendor build vs. buy decisions — translating complex, ambiguous problems into defined solutions
  • Drive alignment across clinical, compliance, growth, and technology teams; hold stakeholders accountable for delivery against defined milestones
  • Define success metrics, track progress, and maintain visibility into risks and outcomes across the initiative portfolio

Required Qualifications:

  • 6–10+ years in strategy, operations, management consulting, or high-growth environments – particularly in a healthcare environment
  • Bachelor’s degree in healthcare, finance, or related field required; Master’s degree preferred
  • Proven track record leading complex, cross-functional initiatives from design through delivery
  • Strong ability to structure ambiguity, align stakeholders, and drive outcomes
  • Excellent communication and executive presence — comfortable presenting to and influencing senior leadership
  • Equally at home in strategic design and in the details of execution

Compensation:

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $150,000 - $200,000. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.

At-Will Employment

Employment with Habitat Health is at-will. This means that either the employee or the employer may terminate the employment relationship at any time, with or without cause, and with or without notice, subject to applicable law. Nothing in this job posting or any other company document is intended to create a contract of employment, express or implied, or to alter the at-will nature of employment.

Any written or oral statements to the contrary are not authorized unless made in writing and signed by an authorized representative of the Company.

Because this is a remote position, employment will be governed by the laws applicable to the employee’s work location, and applicable legal requirements will be followed.

Vaccination Policy, including COVID-19

To help maintain a safe environment for our participants and team members, Habitat Health maintains vaccination and infection control policies applicable to certain roles and work locations. For COVID-19 and influenza, team members who are working in a participant care setting are required to provide either proof of vaccination or a signed declination form and comply with applicable masking requirements while in a care setting to the extent permitted by law in the applicable jurisdiction.

Habitat Health will consider requests for reasonable accommodation based on an applicant's disability, medical condition, or sincerely held religious belief, practice, or observance in accordance with applicable law.

As a condition of employment, team members are expected to comply with all applicable infection control protocols, personal protective equipment (PPE) requirements, and vaccination requirements associated with their role and work location, and to the extent required by law and as permitted in the applicable jurisdiction.
