Position Summary:

The Clinical Operations Coordinator plays a key role in supporting the Head of Clinical Operations in driving clinical excellence, standardization and regulatory compliance across multiple home health agencies. This role serves as a bridge between corporate clinical leadership and field operations, ensuring consistent execution of clinical programs, policies and initiatives that promote quality, compliance, and operational efficiency.

Key Responsibilities:

Program Coordination & Implementation

  • Support the rollout of new and existing clinical initiatives, policies, standard operating procedures, and best practices across markets.

  • Coordinate cross-agency projects that enhance care quality, efficiency, and staff engagement.

  • Partner with agency Directors of Nursing (DONs) and field leaders to ensure consistent understanding and adoption of clinical standards.

  • Maintain project trackers and dashboards to monitor progress, performance, and compliance milestones.

Standardization & Process Improvement

  • Assist in identifying opportunities for workflow standardization while respecting local operational differences and regulatory nuances.

  • Document, update, and maintain standard operating procedures (SOPs) for clinical programs.

  • Collect and analyze data to inform decisions around staffing models, productivity benchmarks, and program outcomes at the direction of the Head of Clinical Operations.

  • Support process improvement efforts using quality improvement frameworks.

Regulatory Compliance & Quality Assurance

  • Partner with the Compliance department to monitor adherence to federal, state and accrediting body regulations (CMS Conditions of Participation, Joint Commission, etc).

  • Assist in preparing for and responding to internal audits, surveys, and quality reviews and action planning.

  • Track key clinical quality metrics and assist in creating reports or dashboards for leadership.

  • Help maintain and disseminate clinical policies, ensuring version control and staff awareness.

Communication & Collaboration

  • Act as a liaison between the corporate clinical operations team and field staff.

  • Support the planning and facilitation of clinical leadership meetings, training and workshops.

  • Assist in preparing communications, presentations, and updates for leadership and clinical teams.

Qualifications:

  • Registered Nurse (RN) license required (active and in good standing).

  • Bachelor’s degree in Nursing, Healthcare Administration or related field required; Master’s preferred

  • Minimum 3-5 years of clinical home health experience.

  • Strong knowledge of CMS Home Home regulations, OASIS, and quality improvement processes.

  • Proven ability to manage multiple projects, prioritize tasks, and meet deadlines in a matrixed organization.

  • Excellent written and verbal communication skills.

  • Proficiency with Google docs, slides, spreadsheets and EMR systems.

This is a Full-Time position with an estimated annual salary of $90,000-$105,000 + bonus and benefits.
