The RD Certified Diabetes Educator (CDE) is responsible to teach patient/family skills for diabetes self- management. Will implement proper protocols for diabetes patient management, will evaluate patient needs at appropriate intervals and suggests appropriate management strategies to the patient and/or physician. Will evaluate current practice standards and evidence and develops and maintains resources. Maintains practice at appropriate levels by attending continuing education activities and researching evidence based practice.

Primary Responsibilities:
- Collaborates with physicians, Health Services Coordinators, and nursing staff to identify patient populations requiring intensive patient education programs
- Assists in staff education related to diabetes
- Works with Marketing Department and Health Promotion to develop custom patient education materials and other marketing activities. Collaborates with Patient Services, Nutritional Services, the Foundation and other areas as needed
- Ensures clinic site compliance with organizational, professional, regulatory and governmental standards of care. Translates and implements changes needed with the clinic site to promote clinic compliance with regulatory standards. Determines the intent of standards as they are related to clinic operations and initiates planning and interventions designed to meet them within specified time frames. Stays current with national specialty standards and promotes professionalism through participation in professional organizations and/or utilization of literature
- Establishes, maintains, and monitors diabetes education programs throughout the clinic
- Provides direct teaching and/or management of high-risk patients with diabetes in accordance with an endocrinologist established algorithms
- Care of Patients: Provides nutritional assessment and counseling. Which includes; nutritional consultation for patients, translates complex nutrition information into a simple form to assist the patient manage their diabetes
- Ensures delivery of safe and efficient care. Reviews with health service coordinators perceived problems in patient education resources, staff education needs, resource availability, patient transition problems, financial data, multidisciplinary team functioning and patient and family expectations. Addresses infection control data to determine effect on complications and initiates controls within the environment to decrease/stabilize this factor. Validates with responsible physicians that care provided to patients is consistent with identified medical regimens. Collaborates with various ancillary services to assure appropriate supplies, equipment, laundry, pharmaceutical and computer-related issues and items are available to clinic site at all times for smooth and efficient patient care delivery
- Attends Diabetes Committee meetings and other meetings as assigned
- Maintains Certified Diabetes Educator Certification
- Performs other duties as assigned

Required Qualifications:
- Bachelor's RD from approved dietetic program
- Licensed Dietician (LD), Registered Dietitian (RD) and RD Certified Diabetes Educator CDE)
- 3+ years of related clinical experience, patient education, and program development
- Experience as a Certified Diabetes Educator
- Excellent written and verbal communication skills
- Excellent organizational skills
Preferred Qualifications: - Master's Degree
- Experience in ambulatory care
- Spanish Speaking
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $49,700 - $88,800 annually based on full-time employment. We comply with all minimum wage laws as applicable.
