Under the supervision of the RN Manager, the Care Advocate is primarily responsible for acting as a liaison between high-risk patients and the medical care team/provider. The Care Advocate will work with patients for a pre-defined period. During this time, the Care Advocate will complete telephonic assessments and document/communicate findings back the medical care team. The Care Advocate will assist patients with scheduling appointments/preventative screenings, help with finding applicable community resources, and will coordinate with vendors as necessary

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $16.15 to $28.80 per hour based on full-time employment. We comply with all minimum wage laws as applicable.

Required Qualifications:
- High School diploma or GED and Graduate from an accredited Medical Assistant program or school. 3+ years of experience at a health care insurance company or medical clinic
- EMR use experience
- Good Alpha/Numeric data entry skills with minimal errors
- Proficient with computer applications that include, but are not limited to: Microsoft Word and Excel
- Proven solid multitasking, problem solving & analytical skills
- Proven to be detail oriented with good verbal and written communication skills, interpersonal skills, and ability to work effectively in a fast-paced work environment
- Proven excellent organizational skills
- Proven ability to communicate and collaborate with colleagues, physicians, nurses, health plan staff, and other healthcare personnel
- Proven ability to communicate verbally and in writing with physicians regarding patient needs
- Proven ability to communicate with all levels of Management
- Proven ability to prioritize and work independently
Preferred Qualifications:
- Medical Assistant Certification
- Certified or Registered Medical Assistant
- CPR/BLS or any other relevant healthcare certification
- Training is in-person at Pearland, which can be up to 90 days depending on progress & performance
- 2+ years of experience with customer service/outbound calls
- Experience working with home health or DME
- Medical Background: Health Coach, Community Health Worker, etc
- Proficient with EPIC EMR
- Role will be a remote role with some in-office requirements (meetings and training)
- Must have a quiet, secure place at home and be present and available during business hours
- Solid home internet connection to support remote work
